Overview
This educational activity is a dementia case conference designed for interdisciplinary healthcare professionals. It focuses on improving collaborative care planning, medication management, and person-centered strategies for individuals living with dementia. Through interactive case-based discussions, participants will analyze real-world scenarios, apply evidence-based practices, and develop actionable solutions to address identified professional practice gaps

Objectives
At the conclusion of this activity, learners will be able to:

  1. Increase knowledge on identifying patients with early signs of cognitive impairment(s)
  2. Increase knowledge on developing care plans to help support patients and their families with dementia
  3. Increase knowledge on the referral process and when it is clinically indicated to refer to specialists
  4. Increase knowledge on the different stages of dementia and care needs
  5. Increase knowledge on managing psychiatric symptoms in patients with cognitive disorders
  6. Increase knowledge on differential diagnosis considerations when seeing patients with cognitive impairments who have psychiatric andor behavioral symptoms
  7. Increase the ability to apply this knowledge to working with rural older adults

Accreditation
Northern Light Health is accredited by the Maine Medical Association Committee on Continuing Medical Education and Accreditation to provide continuing medical education for physicians.
